Caution: Big Data Can Have Adverse Effects on Clinical Practice

The rapid introduction of computers into medical systems leads to the accumulation of an abundance of data. These data can, and probably will, alter clinical practice in both positive and less positive ways. I present two examples of possible consequences of the availability of big data on clinical practice and on physician and hospital management decision making. At the level of the clinical decision making there is the possibility to use big data to look for optimal courses of action for a specific patient, based on patient data from large patient populations. This may lead to improved, individualized evidence-based medicine, but it may also lead to strong effects of judgmental biases.
At the level of evaluating physician and staff activities, detailed records of patients and medical staff actions (especially in intensive care) may allow the evaluation of staff actions. However, this may have adverse effects if the evaluated behavior is not the behavior that is most important from a clinical perspective. It seems that the use of information from big data for clinical decisions, as well as for managerial decisions and other applications should be preceded and accompanied by systematic modeling of the human users' (physicians and nurses) actions with these data and the possible consequences different system designs may have on user decisions and actions and on system performance. Otherwise the use of these data may have adverse effects for the medical staff and for patients.
